Lone Star Circle of Care (LSCC) has received the TMF Physician Practice Quality Improvement Award for improving patient outcomes for performance in 2016-2017 in high-priority clinical areas. LSCC demonstrated excellence in health care quality through effective care management and the reliable delivery of preventative health services.
Presented by TMF Health Quality Institute, Texas Medical Association and Texas Osteopathic Medical Association, the award honors physicians practices for improving patient outcomes while applying technologies and strategies to support effective care management and comprehensive preventative health services. LSCC was one of 97 physician practices in three states and the territory of Puerto Rico to receive the award.
The TMF Physician Practice Quality Improvement Award is based on four components:
· Top performance on nationally recognized clinical outcome measures
· Regular use of population care management methods
· Participation in data reporting quality initiatives
· Implementation of tools and processes to promote patient engagement and improve the patient’s experience.
The award criteria aligns with national health priorities established by the Centers for Medicare & Medicaid Services to improve patient care and satisfaction, promote effective care management and encourage preventative health services, including patient education, early detection and chronic disease management.
