Your Medicare Annual Wellness Visit is a yearly visit focused on keeping you healthy. It is a chance to talk with your care team about your health, risks, medications, and preventive care needs.
Before or during the visit, you may be asked to complete a short Health Risk Assessment. This helps your care team create or update a prevention plan that is right for you.
- Check your height, weight, blood pressure, and body mass index (BMI).
- Review your medical history and family health history.
- Review the medicines, vitamins, and supplements you take.
- Talk about your safety at home, including fall risks.
- Check for memory or thinking concerns.
- Talk about healthy habits, such as activity, nutrition, and tobacco or alcohol use.
- Discuss advanced directives, if you choose. These are documents that share your wishes for future medical care if you cannot speak for yourself.
- Give you a written prevention plan, including recommended screenings, vaccines, and other preventive services.
Please note: The Annual Wellness Visit is focused on prevention and planning for your health. If you have a specific concern or symptom, we may ask you to schedule a separate visit to make sure your needs are addressed appropriately.

This visit allows your care team to identify health concerns early. It also provides an opportunity to coordinate your care effectively through a proactive approach.
Cost of a Visit
- Medicare Part B covers the Annual Wellness Visit once every 12 months.
- You usually pay nothing if your provider accepts Medicare assignment.
- You may have a cost if additional tests, treatments, or services are provided during the visit.
