The following is a summary of the benefit programs. This is not a plan document. For complete plan eligibility, rules, exclusions and requirements refer to the appropriate plan document and policy. Where the plan document and/or policy is not the same, the plan document governs.

A Guide to Lone Star Circle of Care Employee Benefits

Lone Star Circle of Care (LSCC) offers insurance benefits to employees scheduled to work 30 hours a week or more. Employees are eligible for the benefits on the first of the month after sixty days of employment. Employees may add their spouse or qualified domestic partner and children to the plan at an additional cost.

The insurance benefits plans include:

Health Insurance

We offer two Preferred Provider Plan options. This allows employees to select the employee-paid premium and corresponding deductible, office copay, and maximum out-of-pocket benefit plan that best meets their needs. Both plans offer in-network and out-of-network benefits

Both plans include a Virtual Visit by computer or mobile device for certain medical conditions. The physician can write prescriptions. There is an office copay.

A separate Prescription Drug benefit is provided with a deductible and a copay based on a Tier program providing a 30-day supply of medication for in-network providers and out-of-network providers. Employees will be responsible for out-of-network provider charges above the usual and customary fees. We offer a mail-in-pharmacy program that supplies a 90-day supply of medication for a two-month copay.

Dental Insurance

We offer a dental plan with in-network and Out-of-network benefits. The plan has an individual deductible (waved of preventive care) and a separate family deductible. Preventative and Diagnostic work is covered at 100%, Basic Services at 80%, and Major Services at 50% in-network and out-of-network. Employees will be responsible for out-of-network provider charges above the usual and customary fees.

The plan includes Orthodontia for children at 50% with a lifetime cap of $1,500.

The calendar year maximum is $2,000.

Vision Insurance

We offer a vision plan with in-network and out-of-network benefits. In-network covers lenses in full (including medically necessary contacts), an exam with a copay, and a frame allowance. Out-of-network has preset allowances for lenses, exams, contacts, and frames.

Exams, lenses, and contacts are covered once every 12 months and frames once every 24 months.

Flexible Savings Account (FSA)

We offer a Flexible Spending Account. Employees’ monthly contributions are deducted from their pay on a pre-tax basis and may be used to pay for qualified health care expenses. Qualified expenses include deductibles, copays, Lasik eye surgery, prescription drugs, and orthodontia.

Employees may contribute up to $2,500 a year. Expenses must be incurred during the plan year. Up to $400 may be rolled over into the next plan year if the employee joins the Flexible Savings Account in the following year. Any amount over $400 will be forfeited as per federal law.

Employees may use a debit card to pay the expenses. Expenses may be paid from the account prior to the employee funding the contribution.

Life Insurance

We offer 2X annual earnings to a maximum of $500,000 Basic Life Insurance and Accidental Death and Dismemberment Insurance. Amounts over $400,000 require Evidence of Insurability. This plan is portable upon termination.

Short-Term Disability Insurance

We offer a Short Term Disability program paying up to 60% of weekly gross wages up to $1,500 a week for eleven weeks. The plan has a fourteen-day elimination period. There are no exclusions for pre-existing conditions. The plan includes a Rehabilitation and Return to Work Program.

Long-Term Disability Insurance

We offer a Long Term Disability program paying up to 60% of monthly gross wages up to $5,000 a month ($10,000 a month for Providers) for eleven weeks. The plan has a ninety-day elimination period. There are no exclusions for pre-existing conditions. The plan includes a Rehabilitation and Return to Work Program.

Employee Assistance Program

This program is a comprehensive resource designed to provide fast and convenient answers and advice on a variety of topics ranging from everyday problems to severe situations. It is available to employees and their family members. Professionals are available 24 hours for advice and three face-to-face session per issue when needed.

Paid Time Off

Vacation Time

Full-time employees, scheduled to work 40 hours a week earn 120 hours per year of vacation time. Employees scheduled for 20 hours or more but less than 40 hours earn pro-rated vacation time. Employees with five years of service accrue vacation time at the rate of 160 hours per year. Vacation Time is accrued each pay period. Up to 80 hours per year of unused vacation time can be carried over into the next year’s vacation. Hours above 80 are transferred to an Extended Leave Account.

Holidays

LSCC grants all regular full-time and part-time employees holiday leave for the following recognized holidays:

  • New Year’s Day
  • Memorial Day
  • Independence Day
  • Labor Day
  • Thanksgiving Day
  • Friday after Thanksgiving
  • Christmas Day

Personal Leave

Full-time employees who work thirty-two (32) hours or more per week will be provided twenty-four (24) hours of personal leave at the beginning of the calendar year. Such time is prorated for employees who come on board during the calendar year.

Part-time employees who work at least twenty (20) hours per week will be provided twelve (12) hours of personal leave at the beginning of the calendar year. Such time is prorated for employees who come on board during the calendar year.

Sick Leave

Full-time employees who work thirty-two (32) hours or more per week will be provided forty-eight (48) hours of sick leave at the beginning of the calendar year. Such time is prorated for employees who come on board during the calendar year.

Part-time employees who work at least twenty (20) hours per week will be provided twelve (24) hours of sick leave at the beginning of the calendar year. Such time is prorated for employees who come on board during the calendar year.

Bereavement Time

Bereavement time of paid bereavement leave will be limited to three (3) days per event and will be paid in accordance with the employee’s full-time or part-time work schedule and paid at the regular rate of pay.

Civic Duty leave

Lone Star Circle of Care encourages employees to fulfill their civic responsibilities by serving jury duty when required. Full-time employees who are summoned for jury service will be granted up to ten (10) days paid leave of absence over a one (1) year period, while part-time employees will be provided up to five (5) days paid leave of absence over a one (1) year period.

Additional Benefits

Retirement Benefits

All Employees may join a 403(b) Retirement Savings plan. The plan has many different investment options. Lone Star Circle of Care may contribute to the plan at the end of the year if funds allow.

Training and Development

Lone Star Circle of Care will reimburse employees for the cost of training classes and the cost to maintain certifications in relation to their position. The eligible amount is limited and dependent upon position and management approval.

Vendor Discounts

  • Discounts at Golds Gym and the YMCA of Williamson County.
  • AT&T offers a 10% discount.

Voluntary Life and AD&D

Employees may purchase Voluntary Life Insurance in increments of $10,000 up to the lesser amount of $250,000 or 500% of gross earnings. Purchases above $150,000 require Evidence of Insurability.

Employees can purchase Voluntary Life Insurance in increments of $10,000 up to the lesser amount of $250,000 not to exceed the employee’s Voluntary Life amount. Purchases above $30,000 require Evidence of Insurability.

Employees can purchase Voluntary Life Insurance in increments of $10,000 for each of their children or Domestic Partners’ children over 6 months to a maximum of $10,000 not to exceed the employee’s Voluntary Life amount. Children ages 14 days to 6 months are eligible for $500.

All life insurance policies are portable.
