This Privacy Policy and Terms of Service agreement discloses the privacy practices for this website (“Website”). This Website does not collect or contain health information about Lone Star Circle of Care patients.

For information about how we handle patient health information please review our Notice of Privacy Practices. The Patient Portal is not part of this Website and is also governed by our Notice of Privacy Practices.

Protecting your privacy is important for Lone Star Circle of Care. We realize that health is a personal, private subject matter, and we want you to be comfortable while visiting our Website. This privacy policy applies solely to information collected by and through this Website. It will notify you of the following:

  1. What personally identifiable information is collected from you through the Website, how it is used and with whom it may be shared.
  2. What choices are available to you regarding the use of your data.
  3. The security procedures in place to protect the misuse of your information.

This document is also an agreement by you regarding the terms and conditions of your use of the Website. Information Submitted to Our Website

When you visit our site, we may ask you to give us your name, e-mail address, telephone number and other pertinent personally identifiable information. This information is considered “Personal Information”, and we may use it to contact you. We will collect this information from you only if you voluntarily provide it. Providing this information is not necessary in order to use the Website.

While visiting our Website, you may have the option to select whether you’d like to receive more information from Lone Star Circle of Care. If you select this option, we may send you e-mails (“Commercial Emails”) introducing a new service or information that we think might be of interest to you. We may also request Personal Information from you in connection with your use of, or participation in, requests for suggestions, customer feedback, and surveys. If you send us an e-mail with questions or comments, we may respond using the information you provided.

If, at any time, you decide that you no longer wish to receive Commercial Emails from Lone Star Circle of Care, you may opt-out by sending us an e-mail, letter or by calling us at the Contact Us information below. Opting out from Commercial Emails will not affect other e-mails you have consented to receive from us, such as responding to a request or receiving health education updates and appointment reminders. and we may still communicate with you in other ways, such as text messages you’ve opted to receive. If you later subscribe to Commercial Emails from us again, that will terminate your prior opt-out. However, you can always opt-out again later.

Information We Collect from Our Website

In addition to any Personal Information or other information that you choose to submit to us the website, we and third parties may use a variety of technologies that automatically (or passively) store or collect certain information whenever you visit or interact with our site (“Usage Information”). This Usage Information may be stored or accessed using a variety of technologies that may be downloaded to your personal computer, browser, laptop, tablet, mobile phone or other device (a “Device”) whenever you visit or interact with our website. Otherwise we do not treat it as Personal Information.

A Device Identifier is a number that is automatically assigned to your Device used to access the Website, and our computers identify your Device by its Device Identifier;

  • the URL that referred you to our website;
  • the areas within our Website that you visit and your activities there, including remembering you and your preferences;

  • your Device characteristics; and

How and When Do We Share Information with Third Parties?

Cookies and Tracking Technologies

We may use various methods and technologies to store or collect Usage Information (“Tracking Technologies”). ETag, or Entity Tag. A feature of the cache in browsers. It is an opaque identifier assigned by a web server to a specific version of a resource found at a URL. If the resource content at that URL ever changes, a new and different ETag is assigned. Used in this manner ETags are a form of Device Identifier. ETag tracking may generate unique tracking values even where the consumer blocks HTTP and/or HTML5 cookies.

Recognition Technologies. Technologies, including application of statistical probability to data sets, which attempt to recognize or make assumptions about users and devices (e.g., that a user of multiple devices is the same user).

At this time, we do not display third party advertisements on our Website. However, we may associate Tracking Technology with you when you are on our Website to enable us to send you ads about us when you are on third party sites or services, and we may use third-party analytics and other service providers to evaluate and provide us and/or third parties with information about the use of the Website and viewing of ads and of our content. If we use Facebook “Custom Audience” services, you can hover over the box in the right corner of such a Facebook ad and find out how to opt-out with us from such “Custom Audience” list. If you opt-out from Facebook “Custom Audience” services, we will remove the matching Personal Information from that list, however, this will not take effect immediately and if you have multiple e-mails or other accounts you may have to opt-out separately for each one. If we use Twitter conversion tracking and tailored audience products, you can opt-out through your account settings on Twitter. If you opt-out from Twitter conversion tracking and tailored audience products, you will be removed from that list, however, this will not take effect immediately and if you have multiple e-mails or other accounts you may have to opt-out separately for each one. Age Policy

We are a general audience service and do not use the Website to knowingly collect personal information from children under the age of thirteen (13) that requires parental notice and consent under the Children’s Online Privacy Protection Act (“COPPA“) without such parental consent.
