Lone Star Circle of Care’s Community Resource platform will eliminate health inequalities to facilitate wellness and optimal living for all, helping to strengthen our communities. To be true to our mission and vision, we need to both assess social determinants of health needs and respond to them.
Lone Star Circle of Care employs Community Health Workers and Case Managers that support our patients by finding community resources that go beyond health care services. They serve as a bridge between our communities, clinic locations and social services by using outreach tools, education and lived experience to increase public health knowledge and address unmet needs outside of the clinical setting.
